Advanced Care Directive Training

July 10th, 12:15pm-2:30pm (Lunch provided)

We encourage anyone 18 and older to attend this training led by our very own Joe Wheatley  in order to complete your Advanced Care Directive. Advance care planning is the process of planning your health care decisions now in case you are unable to speak for yourself in the future. We will be following the MyCare document to help you understand and honor your future health care goals, discuss end of life issues, and to discern a person that you would want to make healthcare decisions for you. Filling out this document is a part of wise living and can be a great gift to your family. Lunch will be provided for all who attend and childcare will be provided if you RSVP to childcare@fmcsb.org by July 6th.

Joe Wheatley has been facilitating caregiver support groups for over 15 years, is a board member for Friendship Adult Daycare Center, former Geriatric Care Manager and Long Term Care Ombudsman. He has served as a Member of Bioethics committee at Cottage Hospital for the last sixteen years, is a Sansum Clinic and Alliance for Living and Dying Well trainer and teacher of Five Wishes and now MYCARE documents for Advanced Directives.
